Responsibilities Include but are not limited to:<ul><li>Provides direct care services to program guests including linkages to community resources and education on alternative methods of coping skills that can be utilized during acute crises to reduce hospitalizations.</li><li>Complete documentation such as Pre-Registration Questionnaire, Individual Service Plan, Functional Assessment, Discharge Plan, and Follow-up documentation. The counselor provides a variety of services, ranging from the teaching of skills to delivery of concrete services to providing counseling to the clients.
