Job Summary
The Nephrology APP functions throughout the system to care for the needs of nephrology patients across the adult continuum. The APP is an integral member of a multidisciplinary healthcare team.
The APP works in many settings, both inpatient, ambulatory, and outpatient dialysis units, to manage the acute and chronic needs of this population, including possible procedures, like point-of-care ultrasound. Specifically, this APP would focus on patients in the Bone and Mineral Metabolism Clinic services.
In addition, the APP will take occasional night, weekend, and holiday coverage. This APP is generally service-aligned, works closely with the Nephrology medical team, and is responsible for ensuring an environment of safe, quality care.
The APP should have a strong foundation in evidence-based practice and education and should demonstrate an advanced level of medical and nursing knowledge, clinical and technical competency, sound clinical judgment, professionalism as it relates to interpersonal and general communication skills, timely and compliant documentation, and should take responsibility for ongoing professional development and competency validation.
