NeighborHealth is seeking an OB Care Navigator to support pregnant patients by identifying health-related social needs (HRSN), addressing care coordination gaps, and connecting patients to clinical, behavioral health, and community-based services. What You'll Do
The OB Care Navigator works closely with OB providers, multidisciplinary care teams, and community partners to ensure patients receive timely, culturally responsive, and coordinated care throughout pregnancy. The OB Care Navigator supports pregnant patients throughout pregnancy by addressing social, behavioral health, and care coordination needs to reduce barriers to care and improve maternal health outcomes.
The successful candidate will be able to perform the following responsibilities:
- Engage pregnant patients throughout pregnancy to assess health-related social needs (HRSN)
- Develop and document individualized care plans in collaboration with OB providers and the multidisciplinary care team.
- Educate patients on navigating healthcare services, insurance coverage, and community-based resources.
- Coordinate referrals and facilitate warm hand-offs to internal services and external community and legal partners.
- Track referral completion and conduct follow-up outreach to ensure patients successfully access needed services.
- Review reports to confirm patients missing initial prenatal and postpartum visits and communicate with perinatal teams regarding outreach
- Participate in weekly high-risk OB rounds to support case review, care planning, and follow-up.
- Document patient interactions, referrals, and outcomes accurately in EPIC.
- Collect and track screening and referral data to support quality improvement initiatives, reporting, and grant requirements.
- Collaborate with Information Technology and Population Health/Quality Improvement teams to support workflow improvements and documentation processes.
- Maintain accurate, standardized documentation to support HRSN care navigation workflows and quality improvement efforts.

What You’ll Bring
- Bachelor’s degree in healthcare related field required.
- Minimum of 1–2 years of experience in care navigation, community health, case management, or OB care coordination. Experience working with underserved populations.
- Strong interpersonal and communication skills, with the ability to build rapport across diverse teams.
- Exceptional problem-solving and conflict-resolution abilities.
- Ability to handle sensitive and confidential matters with professionalism and discretion.
- Proficiency in Microsoft Office Suite.
- The ability to work in a fast-paced environment.
- Spanish fluency strongly preferred; ability to communicate effectively with a predominantly Spanish-speaking patient population.
Work Environment & Physical Requirements
- This position operates in a professional office and clinic environment.
- Regularly required to sit, stand, walk, and use hands to operate a computer and other office equipment.
- Must be able to remain in a stationary position for extended periods while performing administrative and computer-based tasks.
- Must have adequate visual acuity, hearing, and manual dexterity to perform essential job functions.
- May occasionally move between clinic and administrative areas and be exposed to a typical clinical setting.
- Expected to follow all NeighborHealth and OSHA safety policies and procedures, including maintaining a safe and ergonomic work environment.
Hourly Rate: Starting at $24.15 per hour, and up to $36.50 based on relevant years of experience.
