Job Responsibilities
PH RN 1-Under supervision, provides professional nursing care in accordance with nurse protocols, policies and/or procedures. to determine eligibility for program.
PH RN 2-
Under general supervision, provides professional nursing care in accordance with nurse protocols, policies and/or procedures. Focuses on the prevention of illness, injury or disability, the promotion of health and maintenance of the health of populations. Cares for population with special needs during disasters/emergencies and biological and chemical protocols.
PH RN 3-
Under broad supervision, provides professional nursing care in accordance with nurse protocols, policies and/or procedures. Focuses on the prevention of illness, injury, or disability, the promotion of health and maintenance of the health of populations. Cares for population with special needs during disasters/emergencies and biological and chemical protocols.
Preferred Experience:
- Provide case management services for children with special needs age's birth to 21 years of age.
- Coordinate on site and off site children's medical services clinics.
- Assists with other clinics as needed.
- Pre-authorize CMS covered services for enrolled clients.
- Identify community resources to meet needs of clients and families.
- Conduct home visits for new and enrolled clients monthly to ensure plan of care is being followed.
- Conduct intake visit to determine eligibility for program.
- Participate in referral committee.
- Participates in gathering statistics for monthly/quarterly reports
Minimum Qualifications
PH RN 1-
Associate degree in nursing from an accredited college or university and completion of a PHN course within one (1) year of hire and current Georgia Registered Professional Nurse license and CPR certification.
PH RN 2-
Associate degree in nursing from an accredited college or university and one (1) year of nursing experience and completion of a PHN course within one year of hire and current Georgia Registered Professional Nurse license and CPR certification; or one (1) year of experience at the lower-level PH Registered Nurse 1 (HCP190) or position equivalent.
PH RN 3-
Associate degree in nursing from an accredited college or university and one (1) year of nursing experience and completion of a PHN course within one year of hire and current Georgia Registered Professional Nurse license and CPR certification; or two (2) years of experience at the
lower-level PH Registered Nurse 2 (HCP191) or position equivalent.
Preferred Qualifications:
- Prior experience working with children with special needs.
- Prior Public Health experience a plus.
- Prior case management experience or prior experience working with community resources
