Role: The Librus CNA or Home Health Aide (HHA) is essential in delivering hands-on, compassionate care to ISNP members in skilled nursing facilities, communities, or private residences. Working in close partnership with the RN Care Coordinator and interdisciplinary team, this individual provides daily personal care, emotional support, and comfort-focused services aligned with the Librus Model of Care.

This role includes end-of-life and high-needs duties, including presence during decline and active symptom episodes, without formal hospice enrollment. The CNA/HHA ensures patients receive respectful, attentive care that enhances dignity and quality of life during vulnerable stages of aging and illness.

Qualifications:
Required:

  • Active certification as a CNA or HHA in the state of Florida.
  • Certification of completion of Alzheimer's Disease and Related Dementias Training through the Florida Department of Elder Affairs.
  • Minimum of 1 year of experience providing hands-on care in an SNF, home health, hospice, or long-term care setting.
  • The ability to provide physical care, emotional presence, and dignity-based support.
  • Strong communication and time management skills.

Preferred:

  • Experience working with elderly or chronically ill populations.
  • Comfort with end-of-life support and palliative care environments.
  • Familiarity with SNF settings and interdisciplinary team workflows.
  • Bilingual a plus

Job Responsibilities:
Personal Care & ADL Support:

  • Assist patients with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, feeding, and mobility.
  • Ensure patient safety and comfort while promoting independence when appropriate.
  • Monitor and report changes in condition or behavior to the RN Care Coordinator promptly.

Comfort-Focused, Whole Person Support:

  • Provide gentle, compassionate presence during times of active illness or decline.
  • Offer supportive touch, emotional presence, and comfort-based care interventions.
  • Support patients and families during advanced illness episodes, respecting cultural and personal end-of-life preferences.

Model of Care Integration:

  • Carry out care tasks in alignment with the Librus Model of Care, emphasizing proactive, person-centered support.
  • Collaborate with SNF staff, Providers, and RN Care Coordinators to ensure seamless care delivery.
  • Support the implementation of individualized care plans prioritizing dignity, symptom relief, and emotional well-being.

Documentation & Communication:

  • Accurately document all care activities and observations per organizational standards.
  • Communicate clearly and compassionately with patients, families, facility staff, and the Librus clinical team.
  • Participate in team meetings and care plan discussions as needed.

Compensation Pay Range:

$16.48 - $23.69
