We are seeking aCommunity Health Specialist 2(CHS2) with a desire and experience working with the to provide field services/case management to program participants, including connecting participants to social support services, advocacy, peer support, and attending primary care and other appointments with clients. You will work alongside the Transition and Community Services Program to support health education and outreach, testing, vaccinations and supporting clients transition safely back into the community.

As a Community Health Specialist 2 in Corrections Health, you will help serve in our three Correctional Health facilities as well as the Mead Building .

1. The Multnomah County Detention Center (downtown)

2. Inverness Jail (near PDX airport)

3. The Juvenile Detention Center (NE Portland)

4. Mead Building (downtown)

This recruitment is will be used to fill the following positions:

  • Limited Duration: A Limited Duration Assignment is expected to last no longer than two years.

As theCommunity Health Specialist 2, other duties will include:

  • Partnering with Public Health and Corrections Health Leadership to create appropriate learning materials for adults and youth in custody in Multnomah County.
  • Leading in-person and on-site client education efforts within the jails and detention centers to provide information, suggest solutions, answer questions, and rely on own lived experiences to inform and educate on healthy behaviors and resources available.
  • Assisting with screening, testing, vaccination or other prevention efforts within the jails and detention centers of Multnomah County.
  • Collaborating with team members on best practices for gathering and sharing information with adults and youth for prevention methods that support the client's needs.
  • Assisting in the coordination of health care, mental health, substance abuse, and social service treatment of participants post-release, including making appointments for clients.
  • Conducting outreach to groups or organizations that work with or support those going into or out of correctional facilities.
  • Partnering with family members and support members to provide team approach in support of clients.
  • Ensuring clients being released are offered housing options as appropriate and help coordinate housing with community partners to continue wellness activities and behaviors.
  • Maintaining necessary written documentation of all activities including documentation in the clients electronic medical record via Epic and Google platforms as appropriate.
  • Guarding and securing private health information and entering in the appropriate system and manner to assist with reports and documentation standards.

TO QUALIFY:

Minimum Qualifications/Transferable Skills*:

Education: Associate's degree AND

Experience:One (1) to four (4) years of experience in community outreach services providing health information, advocacy, social support and assistance in using the health care system to groups and families.

(Equivalency 3-6 years of qualifying training and/or experience).

Other:

  • The ability to pass a Multnomah County Sheriff's Office background check / investigation, which will include being fingerprinted. Employment is contingent on passing a background investigation.
  • Must have valid CPR / BLS Certification upon hire OR within 30 days of hire.
  • The employee holding this position is designated as an Essential Employee under Multnomah County Personnel Rule 3-15-015, Essential Employees.
