Under general to limited supervision provides community health services for children birth to age 5 within the 12 counties served by District 4 Public Health. Provides developmental screenings and patient education, advises caregivers, and makes necessary referrals for other services. Works in collaboration with public health and local community human service agencies to provide person-centered support to individuals and families; to help build client capacity to increase knowledge and self-sufficiency. Provides outreach, support, and education to the target population and their surrounding community to reduce disparities in birth outcomes.
All District 4 Public Health, County BOH employees are deemed essential employees in the event of an emergency. As such, you may be called on to participate in an event not limited to but including, staffing a shelter, a Specimen Point of Collection Site (SPOC) and/or Point of Distribution site (POD). When you are called on to participate, you are expected and it will be a part of your job requirement to participate, whether the emergency occurs during or outside regular work hours.
Job Responsibilities
- Provides developmental screening to all children from birth to age five years old who have not been referred with a previously completed developmental screening.
- Provides virtual assessments, parent education, and developmental monitoring according to Children 1st model of service delivery.
- Refers identified infants and children to public health and community programs as appropriate.
- Ensures that all children receiving services are linked to a primary healthcare provider.
- Assures that all identified families receive Children 1st Maternal Child health (MCH) Integrated assessments according to programmatic guidelines.
- Completes all necessary documentation – written and electronic – and reports requirements by specified deadline or as determined by Children -1st District Coordinator.
- Enters follow up actions and assessments into SENDSS Newborn database.
- Checks SENDSS and/or maintains a tickler system to monitor children assigned.
- Participates in health fairs and educational programs.
- Prepares to respond to public health emergencies as directed.
- Communicates in writing, orally, and through electronic means, with proper terminology and cultural proficiency.
- Adheres to the organizations policies and procedures.
- Provides general clerical support to District 4 Child Health Programs.
- Occasional travel throughout the district’s 12 counties for training and program support as necessary.
Minimum Qualifications
Social Services Tech 3
High School diploma/GED AND three (3) years of job-related experience in a social service-related position; OR One (1) year of experience at the lower-level Social Services Tech 2 or equivalent positionAdditional Information
Preferred Qualifications:
- Public Health experience
- Experience with screening and interviewing patients
- Bilingual Spanish/English Speaking Language
- Proficiency in Microsoft Office Suites
- Caseload management and organizational skills
- Detail oriented and excellent follow-through skills
- Good phone etiquette, message-taking, and time management skills
