Job Responsibilities
Under broad supervision, interviews and investigates applicants and recipients to determine eligibility for use of social programs and agency resources. Provides a broad range of social services to patients/clients/consumers and their families for the Ryan White/ HOPWA Program (Housing Opportunities for Persons Living with AIDs). Conducts patient/client/consumer intakes, participates in treatment team planning, and performs crisis intervention. Assists in dealing with personal and social problems. May provide supportive services to clients and families and/or serve as a liaison for social services. May perform case management duties. May serve as lead worker.
TYPICAL TASKS:
•Provides a broad range of paraprofessional social services to clients according to established procedures.
•Serves as liaison between Ryan White and HOPWA (Housing Opportunities for Persons with AIDS) Program, clinical services, and partners.
•Provides case management based on interviews of patient, client and consumer intakes, their families, significant others and appropriate community agencies.
•Recruits new clients and provides group sessions for the Ryan White/ HOPWA Program.
•Documents client follow-up in client record database.
•Promotes increased community awareness of the Ryan White/ HOPWA Program and other services offered at DeKalb Public Health.
•Performs additional duties as assigned
Minimum Qualifications
High school diploma/GED and three (3) years of job-related experience in a social services-related position
OR
One (1) year of experience at the entry level Social Services Tech 2 (SST011) or equivalent position.
PREFERRED QUALIFICATIONS:
•Has a basic knowledge about HIV/AIDS
•Culturally competent and able to work effectively with diverse populations
•Possess exceptional customer service and interpersonal skills with staff, co-workers, and community partners.
•Has effective written & verbal communication Skills
•Proficient in using Microsoft Windows and Microsoft Office Applications, including Excel, Word, PowerPoint, Outlook; Proficiency with Internet
•Proficient in the use of and/or experience with client record database systems
•Ability to operate standard office equipment including computers, fax machines, copiers, printers, telephones
