Department: Provider RelationsReports To: Manager, Provider Relations and Value Based Performance
Position Summary:
The Market Development Liaison – Home Health is responsible for developing strategic relationships with hospital systems and post-acute (PAC) providers, including Skilled Nursing facilities, inpatient rehab facilities, and LTACs. This role will support the optimization of patient transitions into home health care.
Key Responsibilities:
- Serve as the primary point of contact between tango and upstream referral partners to foster strong, collaborative working relationships
- Educate providers and their staff on referral submission processes, submission options and support internal team on care management platform connectivity
- Conduct outreach, presentations, and follow-up meetings to maintain visibility and drive adherence to the referral process
- Identify and troubleshoot barriers to successful referrals; escalate issues to internal teams as appropriate to ensure resolution
- Coordinate with upstream provider key stakeholders such as case management, social work, home health liaison, office manager etc., to support smooth patient transitions to our referral platform
- Maintain accurate records of provider contacts, meetings, and follow-ups in CRM or tracking tools
Qualifications:
- Associate's or Bachelor's degree in healthcare administration, business, or a related field (or equivalent experience).
- 2–5 years of experience in provider relations, healthcare education, physician liaison, or referral coordination.
- Excellent interpersonal, communication, and presentation skills.
- Proficiency in CRM tools, Microsoft Office, and virtual meeting platforms.
Preferred:
- Experience working with upstream referral providers (e.g., hospitals, primary care, Eds, primary care practices).
- Knowledge of home health care operations and referral workflows.
- Familiarity with discharge planning platforms.
