Position Summary:Medical Assistant – Preventative Health Clinic Location: Revere Health Department: Preventative Health & Wellness Clinic Reports to: Clinical Supervisor / Preventative Health Manager Position Summary: The Medical Assistant (MA) in the Preventative Health & Wellness Clinic plays a vital role in improving patient outcomes through proactive outreach, preventive care coordination, and quality gap closure. This position focuses heavily on patient engagement via phone, collaboration with payers and clinical teams, and supporting providers to deliver Annual Wellness Visits (AWVs), screenings, and preventive services.
Essential Job Functions: Key Responsibilities: Patient Outreach & Engagement Conduct extensive outbound calls to schedule Annual Wellness Visits (AWVs), screenings, and preventive health services. Contact patients with open quality or care gaps to ensure completion of labs, screenings, and follow-up appointments. Educate patients on preventive health benefits, available resources, and care coordination opportunities. Document all outreach activity and patient communication in the EMR accurately and timely. Care Coordination & Gap Closure Review patient lists from payers and internal dashboards to identify open care gaps. Coordinate with primary care providers (PCPs), specialists, and payers to close quality gaps and improve patient outcomes. Track preventive metrics (e.g., colon cancer screenings, mammograms, A1c testing, immunizations). Support providers and clinical staff in completing necessary preventive services during patient encounters. Collaboration & Communication Work closely with payers’ quality teams to align efforts on shared patient outreach lists and reporting. Partner with internal Value-Based Care (VBC), population health, and clinical teams to improve overall performance. Participate in regular huddles and team meetings to review outreach progress, barrier trends, and strategies for improvement. Clinical & Administrative Support Room patients and assist providers as needed with preventive visits and screenings. Take vitals, update patient histories, and maintain accurate clinical documentation. Support scheduling, coordination, and patient flow for preventive and chronic care management visits. Follow HIPAA and Revere Health standards for patient confidentiality and data accuracy.
Qualifications: Qualifications: Education: Certified Medical Assistant (CMA), Registered Medical Assistant (RMA), or equivalent certification preferred. Experience: 1+ year of MA experience preferred; experience in preventative care, population health, or care coordination strongly desired. Skills: Excellent phone communication and patient engagement skills. Strong attention to detail and ability to multitask in a fast-paced environment. Proficient in EMR systems and data tracking for quality metrics. Ability to collaborate effectively across clinical and payer teams.
Hours: 8 to 5 PM, M to F
