Position Summary: We believe in proactive, patient-centered care. Our wound care program is designed to help patients heal faster, prevent hospitalizations, and improve quality of life. We’re looking for a motivated Medical Assistant to join our growing wound care team in Provo—someone who’s hands-on in the clinic and proactive in reaching out to patients in the community. Wound Care supports the wound care team by providing clinical and outreach support to patients with high-acuity wounds and chronic conditions. This position is ideal for someone who thrives in both a clinical and outreach setting, helping patients in person and over the phone to close care gaps and ensure continuity of care.
Essential Job Functions: Responsibilities Clinical Support: • Assist providers and the wound care RN with wound assessments, dressing changes, and treatments. • Prepare and clean treatment rooms, restock supplies, and manage autoclave sterilization. • Obtain and record vital signs, wound measurements, and patient history in the EMR. • Prepare for and assist with debridement procedures. • Perform wound photography and document progress accurately. • Educate patients on wound care instructions, supplies, and follow-up. • Ensure infection control and safety protocols are always followed. Outreach and Patient Engagement: • Conduct proactive outreach calls to patients with open wounds or chronic conditions to ensure follow-up care is scheduled and completed. • Close quality gaps (such as missed follow-up visits, dressing changes, or preventive screenings). • Identify and reach out to high-acuity or at-risk patients to help prevent hospitalizations. • Coordinate with providers, RNs, and the Value-Based Care team to track outcomes and continuity of care. • Assist in maintaining patient lists and outreach documentation in the EMR or population health platforms. • Support patient education and engagement initiatives to improve compliance and healing rates.
Qualifications: • Certified Medical Assistant (CMA), Registered Medical Assistant (RMA), or equivalent certification preferred. • Previous experience in wound care, dermatology, or a high-acuity clinical setting strongly preferred. • Comfortable making a high volume of outreach calls and engaging with patients over the phone. • Strong interpersonal and communication skills. • Excellent attention to detail and documentation. • EMR experience preferred
Hours: 8 to 5 PM, Monday to Friday - 40 hours a week
