Job Summary

This RN role is centered around supporting the patient access workflow, with an emphasis on improving first-call resolution, reducing call transfers, and enhancing response times for patients. The Clinical Nurse acts as a vital clinical resource, addressing patient concerns and escalations when patients first connect with the care team.

Timely clinical triage and assessment of patient needs are key responsibilities, ensuring guidance is provided according to established protocols. By doing so, the RN helps deliver efficient, high-quality care, minimizing unnecessary follow-ups and delays. Collaboration with providers and care teams is integral to this role.

The RN will assist in reducing provider message volume, streamlining communication, and supporting overall clinic efficiency. This position is crucial for improving the patient experience and facilitating the successful implementation of new access workflows within Primary Care.

In addition to the triage duties, the Clinical Nurse may also work directly in the clinic alongside providers when operational needs require additional support.

The Nurse Clinical Ambulatory provides disease specific patient care to a group of patients who have serious acute and/or chronic health events or who are seeking healthcare to maintain their state of wellness. Responsibilities of the position include, but not limited to skilled assessments, care plans and care plan implementation, patient education and clinical evaluation or interventions. This position requires working with medical professionals who are working under time and efficiency constraints in a busy ambulatory care setting with a patient centered care focus on quality, safety, and service. A working knowledge of enterprise, nursing, departmental policies, protocols, and procedures is needed to be successful.
