Registered Nurse (RN) plus national certification in area of specialty, ambulatory care nursing, or care coordination and transition management within 2 years



Physical Requirements

The physical requirements for this position include regularly sitting, standing, walking, with some pushing, pulling, and/or lifting objects weighing up to 10lbs and occasional pushing, pulling, and/or lifting objects up to 50lbs.



Shift

Monday – Friday, Day Shift; primarily 8:00 am – 5:00 pm (40 hrs/wk); other hours/schedule as needed



Job Summary

The nurse clinical coordinator plays a vital role in coordinating patient care, acting as a liaison between patients, their families, and various healthcare providers. Previous experience in cardiology and the care of patients with heart disease is preferred, with demonstrated valvular heart disease-specific knowledge. Will work primarily with valvular heart disease patients and their providers, ensuring appropriate referrals, compliance with medical regimens, coordination of care, and appropriate follow-up. Participates in re-admission reviews and is responsible for data entry. Serves as a point of contact for the valve tracking program.
