Position Overview


The Patient Care Specialist is a key member of our care team, responsible for assisting patients during visits, coordinating follow-up care, and managing remote patient monitoring programs. This role is ideal for someone who is organized, compassionate, and comfortable with both clinical and technology-driven tasks.

Key Responsibilities

  • Welcome and assist patients during in-office visits, including rooming and vital signs.
  • Prepare and maintain exam rooms to meet cleanliness and safety standards.
  • Support providers with procedures and accurate documentation.
  • Monitor patients remotely using approved devices and platforms; track health data and escalate concerns as needed.
  • Conduct follow-up calls to review care plans, medication adherence, and patient progress.
  • Coordinate referrals, schedule appointments, and manage patient flow efficiently.
  • Provide exceptional customer service and address patient questions promptly.

Qualifications

  • High school diploma or equivalent required; additional healthcare training preferred.
  • Previous experience in a healthcare setting or patient coordination role is a plus.
  • Strong communication and organizational skills.
  • Ability to multitask and work in a fast-paced environment.
  • Comfortable using technology for remote monitoring and electronic health records (EHR).
