Role:
The Pharmacist is responsible for the provision of pharmaceutical care, with emphasis on clinical components, to ensure optimal patient outcomes for hospice patients. Supervises technical support staff.

Qualifications:
• Graduation from an ACPE accredited school of pharmacy
• Current licensure as a Pharmacist in the State of Florida
• Experience in the home care or hospital setting preferred
• Computer/data base experience
• Ability to work on-call and shift hours
• Well-developed interpersonal and communication (verbal and written) skills; ability to be tactful and understanding in contacts with patient/families, physicians, volunteers and other Company staff
• Highly organized and precise
• Analytical and thorough in work
• Able to work under moderate supervision exercising a high degree of discretion and independent judgement

Job Responsibilities:
• Ensures the accuracy of all medication orders by checking for the appropriateness of drug selection, dose, frequency, safety, dosage form, drug interactions, allergies and disease state interactions and by consulting with nursing and medical staff prior to dispensing of medications. Ensures the accuracy of all medications dispensed by supervising all technicians’ duties.
• Orients and trains new personnel. Participates in annual performance evaluations.
• Assists the Pharmacy Manager with implementation of written policies and procedures to ensure compliance with federal and state regulatory agencies, Joint standards, hospice policies and procedures. Ensures the proper provision of pharmacy services.
• Participates in Interdisciplinary Group (IDG) process to discuss present patient status, progress by lab study evaluation, direct patient contact, consultation with nursing and physician staff and other appropriate clinical personnel, order changes, recommendations and care plans ensuring patient and customer questions/concerns are addressed appropriately and professionally in a timely manner.
• Assists with development, implementation, monitoring and functioning of Pharmacy software, Pyxis (automated dispensing system), Pharmacy Robot and other systems.
• Ensures the proper use and maintenance of all Pharmacy Department computer/data processing systems and the security and confidentiality of all data contained within such systems.
• Provides appropriate training and periodic evaluation of aseptic technique for employees involved in preparation of sterile products and in the handling of cytotoxic drugs. Supervises IV room cleaning logs and verifies pharmacy temperature logs are completed daily and reviewed periodically to ensure compliance.
• Performs other duties as assigned.
