DUTIES AND RESPONSIBILITES:

  • Oversee coordination of a patient’s care while in PHP, including daily group therapy, 2 individual sessions per week and family/referent updates as needed.
  • Complete clinical assessments (level of care evaluation, biopsychosocial/integrated summary), develop person-centered plans, provide therapeutic intervention and review goals biweekly to determine level of care necessity.
  • Aftercare assessment and planning – assess unresolved and deferred problems and write goals in the aftercare plan and coordinate referral to appropriate aftercare providers.
  • Oversee the collection of rand urine drug screen and breathalyzer samples from patients and relay results.
  • Responsible for documenting treatment plans, progress notes, treatment plan updates, case consultations, telephone calls and discharge summaries in the patient records in a timely fashion.
  • Facilitates group therapy and individual counseling that is consistent with the patient’s treatment goals and their treatment plan.
  • Family updates and sessions as clinically appropriate
  • Responsible for completing necessary utilization review information and forwarding it to the Financial Resource Specialists.
  • Works within the ethical constraints of the certified addiction counselor and other professional or organizational ethical standards as they apply to the position.
  • Document treatment plans, progress notes, treatment plan updates, case consultations, telephone calls and discharge summaries in the patient electronic health record in accordance with Caron policies and procedures.
  • Completes necessary utilization review information.

  • Must be able to deliver presentations that may be both multiple times and for lengthy time periods.
  • Must be able to lift up to 20 lbs.
  • Must be able to move throughout Caron properties.
  • Must be able to ascend and descend stairs.
  • This is a safety sensitive position.

EDUCATION / EXPERIENCE QUALIFICATIONS:

  • Master’s Degree level in social work, marriage and family therapy, mental health counseling, or related behavioral health services.
  • A minimum of 6 months working in the behavioral health field required.
  • Preferred Specialty Training (i.e., MI, DBT, MBSR, CPT, CBT, EMDR, Trauma and Rainbow Advocacy)
  • Experience in lifespan developmental issues for their respective populations.
  • Experience in facilitating group therapy required.
  • Must possess a valid PA driver’s license and must comply with ACT 33 of Pennsylvania law to work with youth populations, when applicable.
  • If in recovery, two years of uninterrupted sobriety preferred.
