Job Responsibilities
The Ryan White Case Manager is responsible for both medical and non-medical case management for adults living with HIV. The case manager is responsible for completing and updating acuity scales, developing and implementing an individualized service plan, and following up with clients as needed to meet health goals. The case manager is also responsible for linking clients to services to help address social determinants of health and barriers to care. The focus of case management interventions is to promote and encourage adherence to care.
Primary Duties:
- Complete initial assessment of service needs by completing the Ryan White Acuity Scale and update per the Ryan White Case Management Standards
- Develop a comprehensive Individualized Service Plan (ISP) for appropriate clients and update per the Ryan White Case Management Standards
- Monitor the client to assess the efficacy of care plan and progress towards health goals
- Ongoing assessment of the client and other key family members’ needs and personal support systems
- Maintain up to date documentation in eClinical (the medical record) for assigned case load
- Document, for all clients, evidence of coordination of services required to implement the ISP during service provision, referrals, and follow-up areas
- Timely and coordinated access to medically appropriate levels of health, support services and continuity of care
- Treatment adherence counseling to ensure readiness for the adherence to complex HIV treatments
- Client-specific advocacy and/or review of utilization of services
- Coordination of prenatal care if applicable
- Other duties as assigned
Minimum Qualifications
Bachelor's degree in a related field from an accredited college or university. Note: Some positions require eligibility for licensure.
Preferred Qualifications:
HIV case management experience is preferred
Master’s degree in social work or a related field preferred
