Description

Concern Housing is seeking a Senior Case Manager to be responsible for assisting the Supervisor in the management and supervision of their program, as well as providing case management services directly to clients.


Duties and Responsibilities:

  • Responsible for providing site on-call coverage as arranged with Coordinator of site.
  • Provide case management services for a caseload of clients: responsible for provision of direct care services, all documentation, attendance at case conferences etc. This may involve working a flexible work schedule.
  • Responsible for coordination of case management services for a caseload of clients. This involves oversight of all services provided as well as documentation and contacts with collateral agencies.
  • Coordination of discharge of clients.
  • With the Supervisor, ensures that program is in compliance with all regulatory guidelines regarding client care, safety and delivery of services, with regard to OMH, Medicaid and CARF.
  • Acts as liaison with other service providers as well as with community agencies.
  • Responsible for maintaining up to date medication documentation
  • Assigning and delegating regular and per diem staff as needed to ensure proper coverage of site.
  • Provides supervision to part time staff, as assigned by site Supervisor.
  • Participates on Agency committees
  • Attends all scheduled staff meetings as scheduled.
  • Participation in regular supervision meeting with supervisor.
  • Perform monthly fire safety inspections.
  • Training and orientation of new staff.
  • Responsible for performing medication counts.
  • Provide housing services including assistance with maintenance issues.
  • Complete annual doctor’s letters and make monthly medication sheets.
  • Remain current with all Agency required compliance documents and trainings.

Salary: $55,000



Position Requirements

Bachelors Degree in Psychology, Rehabilitation, Social Work or other related mental health discipline, plus 3 years experience working in a similar setting and/or working with people with mental illnesses. The individual in this position must have good verbal and written communication skills as well as the ability to make sound judgments regarding client care.
