Position Summary: Revere Health is seeking a dedicated and proactive Social Worker to join our Integrated Behavioral Healthcare Team to support our team-based, patient-centered care model. This role works alongside behavioral health providers, medical assistants, care managers, and primary care teams to help patients access resources, improve follow-up care, and navigate social determinants of health needs. This position does not provide clinical therapy or mental health treatment but plays a key role in ensuring patients receive comprehensive whole-person care.

Essential Job Functions: · Support integrated behavioral health workflows, including screening, scheduling, patient outreach, and care coordination. · Assist in administering and tracking standardized screenings (PHQ-9, GAD-7, SDOH tools, etc.) and notifying clinical staff of results. · Conduct patient follow-up calls to support care plans, appointment reminders, and resource navigation. · Help patients access community resources related to transportation, housing, food insecurity, financial assistance, and other social determinants of health. · Coordinate with behavioral health clinicians, medical providers, and care managers to ensure timely communication and patient support. · Provide non-clinical patient education on clinic processes, wellness resources, self-management tools, and follow-up steps. · Assist with referrals to specialty behavioral health providers, care management programs, and community agencies. · Document patient interactions and care coordination activities in the electronic medical record (EMR) in accordance with Revere Health policies. · Monitor outstanding patient needs, referrals, and follow-up tasks to support value-based care goals. · Participate in team huddles, quality improvement activities, and practice-based meetings. · Maintain a patient-centered, culturally sensitive, and trauma-informed approach in all interactions.

Qualifications: Minimum Qualifications: Bachelor's or Master's Degree in from an accredited school along with required completion of field hours. · Previous experience in a healthcare, or social services setting strongly preferred. · Strong communication, problem-solving, and organizational skills. · Ability to work collaboratively with a multidisciplinary team in a fast-paced environment. · Comfortable discussing social needs, collecting screenings, and working with diverse populations. · Basic computer skills and the ability to learn EMR systems. Preferred Qualifications: Experience in care coordination, medical assisting, patient navigation, or community resource support. · Knowledge of social service agencies, Utah community resources, and SDOH screening workflows. · Bilingual (English/Spanish) skills. · Experience with Epic or similar EMR systems

Hours: Hours may vary based upon clinic hours and patient/provider needs.

Additional Notes: · Work is performed in a medical clinic environment with frequent patient interaction. · Must be able to sit, stand, walk, and assist patients as needed. · Occasional lifting up to 25 lbs. · May require travel between Revere Health clinic locations in Utah.
