Other days and hours as needed.Job SummaryThe Division of Infectious Diseases is seeking a social worker to join its Bluegrass Care Clinic team. They will play a vital role in ensuring access to comprehensive, patient-centered care for individuals living with HIV.
This position is primarily responsible for conducting psychosocial intake assessments for the Ryan White HIV/AIDS Program, determining eligibility, identifying barriers to care, and connecting patients to appropriate medical, behavioral health, and community-based resources. They will also support linkage into care for justice-involved individuals.
As a member of a multidisciplinary healthcare team the Social Worker collaborates closely with medical providers, nursing staff, case managers, and behavioral health professionals to support patient engagement, retention in care, and adherence to treatment. The role requires strong assessment, documentation, and communication skills, as well as a trauma-informed, culturally responsive approach to care for diverse and socially complex populations.
Skills / Knowledge / AbilitiesGood communication, documentation, organizational, and problem solving skills; Experience with Windows PC, Microsoft Office (Word, Excel, Databases); Bilingual (Spanish/English) preferred.Does this position have supervisory responsibilities?
