Physical Requirements

The physical requirements for this position include regularly sitting, standing, walking, with some pushing, pulling, and/or lifting objects weighing up to 10lbs and occasional pushing, pulling, and/or lifting objects up to 25lbs.



Shift

Monday – Friday, Day Shift; primarily 8:00 am – 5:00 pm (40 hrs/wk); other hours/schedule as needed.



Job Summary

The Social Worker will serve as a critical member of the interdisciplinary care team within UK HealthCare’s pediatric specialty ambulatory clinics. This role focuses on identifying and addressing non-medical factors impacting patient health through the use of Social Determinants of Health (SDOH) screening and clinical navigation. The Social Worker collaborates with physicians, advanced practice providers (APPs), behavioral health consultants, nurses, and financial counselors to secure essential resources for patients and families, including financial assistance, transportation, and community-based support services.

The Social Worker will provide direct consultation, education, and emotional support to patients and families, promoting health equity and care continuity. This includes documenting interventions in the medical record, coordinating care to reduce duplication of services, and ensuring families understand and access their eligibility for local, state, and federal services. The position also involves participating in staff meetings and departmental coverage, evaluating the need for DCBS referrals, and offering safety-related counseling, such as access to lethal means mitigation. The ultimate goal is to reduce barriers to care and improve patient outcomes through proactive, collaborative, and compassionate support.
