Job Summary
- The Therapeutic Care Provider II delivers comprehensive therapeutic services in support of hospital programs for individuals with psychiatric or substance use disorders. This role is responsible for intake, clinical assessment, therapy, treatment planning, and care coordination in collaboration with interdisciplinary teams.
Essential Functions
- Conducts biopsychosocial assessments and facilitates appropriate level of care determinations.
- Provides individual, group, and family therapy sessions in accordance with treatment plans.
- Develops, implements, and updates individualized treatment and discharge plans.
- Coordinates referrals and facilitates patient access to community resources and services.
- Supports patient registration and pre-certification processes as needed.
- Integrates evidence-based practices into clinical interventions and care delivery.
- Travels as necessary to conduct patient assessments or support program needs.
- Provides limited nursing or clinical support functions as appropriate within scope.
- Collaborates with interdisciplinary teams and community partners to support continuity of care.
- Participates in outreach and program development activities, including hospital marketing efforts.
- Performs other duties as assigned.

Qualifications
Education
- Bachelor’s Degree in Social Work, Psychology, or a related field with a mental health specialization. (Required)
Licenses/Certifications
- None required upon hire. (Licensure may be required based on facility or state-specific regulations.)
Experience
- Experience in discharge planning, clinical case management, or a community agency setting. (Required)
