TheTransition Services Program Supervisor (TSP Supervisor) oversees the Transition Services Program within Corrections Health, serving justice-involved individuals in Multnomah County. This role supervises a multidisciplinary team (Medical Providers, QMHPs and QMHAs, Community Health Workers, Nurses, support staff) focused on discharge planning and addiction treatment, including Medication Supported Recovery (MSR) services in jail and community connections.
This position emphasizes equity, ensures culturally specific services and monitors program success with an equity lens, adhering to guidelines from the Corrections Health Director and management, developing strategic plans, and maintaining compliance with regulatory and performance guidelines, including those from the National Commission on Correctional Health Care (NCCHC).
The Transition Services Program Supervisoralso fosters a respectful, trusting, and understanding work culture for diverse populations within the Health Department. This essential public safety role requires reporting for duty regardless of County closures.
As the Transition Services Program (TSP) Supervisor, you will:
Leadership and Personnel Supervision
Lead and develop effective teams, motivate staff growth, foster positive relationships, and share vision. Observe practice, skills, client interaction, and documentation. Coach, counsel, and discipline for performance, ensuring continuity of care and compliance with rules. Conduct ongoing and annual performance evaluations. Ensure evidence-based interventions for Corrections Health clients. Hire personnel per Multnomah County rules and collective bargaining agreements. Ensure clinical training and coaching for new staff competency. Coordinate with leadership for staff training and education. Ensure compliance with Health Department guidelines, policies, procedures, and standards (NCCHC, HIPAA, OSHA, etc.).
Program Development
Develop and implement client-linking workflows from jail to community resources post-release. Build partnerships to help clients navigate the justice system and meet health/social needs. Ensure clear discharge planning across facilities. Provide public information, attend relevant meetings, and assist with budget preparation. Evaluate and improve operations, policies, and strategic plans, reporting on activities.
Monitor legislative actions impacting programs, especially Medicaid expansion for those in custody. Collaborate with the Corrections Health Senior Leadership Team and TSP Manager to develop and monitor strategic plans, ensuring regulatory compliance. Oversee grant budget, expenditures, and reporting, authoring reports and monitoring funding compliance with the Transition Services Program Manager and collaborators.
Communication and Community Engagement
Serves as the subject matter expert for discharge planning and transition services. Builds and fosters positive relationships with agencies such as the District Attorney's Office, Department of Community Justice, and the Sheriff's Office to understand their programs and facilitate clients' successful navigation out of the justice system.
Works with community programs to expand the network of client supports, ensuring the availability of culturally specific options. Ensures messaging aligns with communication plans, especially when addressing political situations or discussing Medication Supported Recovery options.
Management Team Member
- Participate as a member of the Corrections Health Leadership Team, managing complex issues with stakeholders. Oversee budget, personnel, program monitoring, and implement changes using expertise in support, budgeting, personnel rules, and union contracts.
- Contribute to the Corrections Health CQI team and provide input as necessary.
- Offer expert technical support to 24/7 staff as the On-Call Manager (2-4 times monthly). This includes making decisions on medical triage (with expert consultation), emergency response, personnel matters, politically sensitive issues, and staffing, such as overtime mandates and call-ins.
Minimum Qualifications/Transferable Skills*:
- Equivalent to a bachelor's degree and three (3) to six (6) years of experience that demonstrates the ability to perform the duties of the position.
(Equivalency 7-10 years of qualifying training and/or experience).
- BLS/CPR certification required
- Must be able to pass a Multnomah County Sheriff's Office Background Check
