SUMMARYEssential Duties and Responsibilities include the following. Other duties and tasks may be assigned.Primary duties include:<ul><li> ESKD rounding, including limited and comprehensive visits, under the guidance of practice nephrologists</li><li> ESKD hollistic patient care management including but not limited to patient activation, depression, high risk of hospitalization, transition of care and co-morbid medical issue oversight</li><li> ESKD patient education</li><li> ESKD comprehensive disease management annual visits</li><li> Care collaboration with with nephrologists, dialysis center IDT, Kidneylink IDT, hospital discharge care coordination team, PCPs and sub-specialisits as clinically indicated</li><li> CKD clinical care and kidney disease education</li><li> Other duties and tasks as assigned</li></ul>Functions performed during above duties:<ul><li> Perform physical examinations of patients to evaluate health and diagnose illnesses</li><li> Order and assess diagnostic tests, such as x-rays and laboratory tests</li><li> Determine, plan, and initiate appropriate treatment, and adjust treatment if necessary</li><li> Prescribe medications</li><li> Document medical information of patients and maintain electronic medical records</li><li> Educate, instruct, and counsel patients and families on presenting health problems, treatment plans, and home care, and encourage and promote preventative health care and positive health behaviors</li><li> Request consultation or referral with other health care providers, such as physicians and clinics, collaborate with Kidneylink RN Care Manager</li><li> Actively promotes GUEST customer service standards; develops effective relationships at all levels of the organization.</li><li> Participates in team concepts and promote a team effort; performs duties in accordance with company policies and procedures.</li><li> Regular and reliable attendance is required for the job.</li></ul>
