Provides social services support to include participant screening, case management, counseling, and referral.</li><li>Participate in the Interdisciplinary Team’s initial assessments, care planning and on going re-evaluations of participant’s case.</li><li>Maintain current written case management records, including periodic reassessments of program participants.</li><li>Provide individual and family counseling, develop and lead group counseling and activities.</li><li>Refer participants to appropriate community agencies or facilities, act as a liaison with such organizations and as advocate for participants.</li><li>Encourage effective relationships amongst staff geared to the building and maintenance of a cohesive team.</li><li>Consult with and advise staff members as to the relationship of social, emotional and cultural factors to health and medical care, and as to the availability of social services in the community.</li><li>Participate in program and policy development of the social work component of the program.</li><li>Represent the company in a positive and professional manner at all times with others, e.g., co-workers, management, vendors, and customers.</li><li>Consistently represents Hope with integrity and professionalism.</li><li>Complete all job responsibilities in a thorough and complete manner, utilizing good judgment in all aspects of the job.
