Care Health Plan's members. This position uses science-based practice to assess, provide nutritional diagnosis, plan interventions based on member goals, monitor progress, and evaluate outcomes. This position documents and communicates interactions with pertinent partners and member's health care team. Additional responsibilities include identifying and collaborating with community-based organizations to develop/coordinate and/or review health education programs. This position is responsible for leading nutrition education initiatives while working with other internal departments such as Quality Improvement and Cultural &amp; Linguistic Services and further assists the Health Education Director and Manager in fulfilling departmental goals.</p><h4>Duties</h4><p>Provide Medical Nutrition Therapy upon receiving a treating provider referral/diet prescription and Diabetes Self-Care Management Education for members referred by internal staff or external providers. Complete medically tailored meals dietary assessments through electronic medical record review, telephonic assessment with members, and facilitation of meal service. Rehabilitation Counselor for the Deaf(Hybrid (Vocational Rehabilitation Counselor 1 (35 Hour)))

<p>SUMMARY</p><p>The <strong>Regional Manager Dietitian</strong> will serve as a liaison between the operations leadership team, the Divisional Lead Dietitian, and the clinical Registered Dietitians of the assigned area. This person will act as a resource and subject matter expert, will support the delivery of excellent nutritional care, and will assist in the improvement of quality outcomes by providing direction and guidance. This person may carry a reduced patient caseload.</p><p>Essential Duties and Responsibilities include the following. Other duties and tasks may be assigned.</p><ul><li>Provides general direction, guidance and feedback based upon professional standards and USRC guidelines to support Dietitians within the assigned area in achieving the desired outcomes in the following: quality, patient satisfaction, teamwork, unit culture, and employee satisfaction.</li><li>Provides communication and training for Dietitians to support clinical competencies and policies and procedures individually and/or through region or division meetings.</li><li>Drives company initiatives with colleagues and Dietitians of assigned area.</li><li>Facilitates communication of continuing education training opportunities to enhance the practice of all USRC Dietitians.</li><li>Coordinates standardization of required documentation for Dietitians (USRC Policy and appropriate documentation to meet CMS Conditions for Coverage).</li><li>Coordinates orientation and training of all new Dietitians within the assigned area with the assistance of the RVP, Facility Administrator, HR department, and/or other disciplines.</li><li>Monitors achievement of USRC quality outcomes withing assigned area and facilitates the development of quality assessment and performance improvement projects.</li><li>Works with operations leadership team to ensure Dietitian coverage assuming responsibility for enlisting Dietitian coverage at dialysis facilities withing the assigned area during times of Dietitians leave of absences, vacations, census increases and vacancies.</li><li>Provides input for the Dietitian's formal performance evaluations and provides feedback throughout the year.</li><li>Attends and participates in USRC leadership team meetings which may include quality, USRC policy, team building, staff development, and other meetings as appropriate.</li><li>Collaborates with appropriate operations management staff as needed to achieve effective inter-disciplinary and intra-disciplinary relationships.</li><li>Actively promote GUEST customer service standards; develop effective relationships at all levels of the organization.</li><li>Participate in team concepts and promote a team effort; perform duties in accordance with company policies and procedures.</li><li>Regular and reliable attendance is required for the job.</li></ul><br>
