OB Nurse Navigator

The Intensive Outpatient Program (IOP) Clinician works within a multidisciplinary team to provide clinical services to individuals with substance and/or alcohol use disorders as part of a new Intensive Outpatient Program at South Shore Hospital. The IOP will provide evidence-based counseling services in group and individual settings for the treatment of substance use disorders as well as co-occurring mental health disorders. This position will also work with the clients to access community resources and work with collaterals in the community, including other behavioral health providers, PCP, and community support groups.&nbsp;JOB FUNCTIONS:<ul><li>Work with a multidisciplinary team of addiction providers, including doctors, advanced practice clinicians, nurses, and recovery coaches</li><li>Assess, diagnose, and complete treatment plans for all clients. Develop treatment goals that are personally meaningful to the client, relevant to the diagnosis, and are behavioral, and measurable.&nbsp;</li><li>Provide person-centered, trauma-informed clinical services to individuals with substance use disorders in an Intensive Outpatient Program (IOP). Clinician will utilize evidence-based practices for substance use disorders and co-occurring mental health disorders including Motivational Interviewing, CBT, and harm reduction.</li><li>&nbsp;Identify appropriate community resources for patients with substance use disorders and match these patients to appropriate services. Skills include demonstrating a comprehensive understanding of the department’s immediate needs and long term growth goals, showing initiative, using critical thinking to problem solve independently, exhibiting leadership qualities, and effectively engaging with the clinical team.Essential Duties &amp; Responsibilities:<ul><li>Assess a patient through obtaining patient history and performing appropriate tests and observations.</li><li>Communicate with the patient’s provider and educate the patient on test results, new care plans and/or other relevant health care information including the use of newly prescribed medications and/or changes in the current medication regime.</li><li>Demonstrates sound clinical judgment/decision making when triaging sick patients, both on the phone and in the waiting room.</li><li>Safely and effectively administers medications, tests and treatments as ordered.</li><li>Teaches patients and families health care regimens. Explains and provides information regarding disease and wellness, tests, results, medications, diet, exercise, and other aspects of the plan of care to patients.</li></ul>Qualifications and Requirements:<ul><li>Bachelor of Science in Nursing preferred.</li><li>Completion of Accredited Nursing Program.</li><li>Licensure in Massachusetts as an RN.</li><li>BCLS certification per department policy.</li><li>1 year relevant nursing experience in a community health and/or ambulatory care setting preferred.</li></ul>
