The Triage Nurse works closely with the Admissions Team and clinical leaders to support timely decisions and facilitate safe and efficient transitions to dialysis care.This is a remote role.&nbsp;Weekly Schedule: Alternates every week between the following shifts: &nbsp; &nbsp;- Shift A: Monday to Friday, 7:00 AM – 3:30 PM central &nbsp; &nbsp;- Shift B: Monday to Friday, 10:30 AM – 7:00 PM centralWeekend Coverage: 1–2 Saturdays per month, 8:00 AM – 4:30 PM&nbsp;centralEssential Duties and Responsibilities include the following. Other duties and tasks may be assigned.<ul><li> Review medical records for new dialysis referrals, focusing on identifying high acuity conditions, missing information, and clinical red flags.</li><li> Complete clinical triage documentation within the internal referral management platform, clearly noting referral concerns, comorbidities, and readiness indicators.</li><li> Communicate effectively within the internal referral management application to notify the Admissions Team when a clinical review is complete and chair time can be released.</li><li> Apply clinical judgment to determine whether the patient is suitable for acceptance into a USRC or SHC facility, based on medical history, current status, and dialysis modality.</li><li> Ensure medical record reviews are completed within the target timeframe of 30 minutes per referral.</li><li> Escalate high-risk or unclear referrals to the appropriate clinical or operational leaders for further review.</li><li> Collaborate with Admissions and field staff to support efficient handoffs and strong communication across departments.</li><li> Maintain current knowledge of internal admission criteria, clinical protocols, and dialysis best practices.</li><li> Participate in training, workflow updates, and quality assurance initiatives as needed.</li><li> Perform other duties as assigned.</li><li> Actively promotes GUEST customer service standards; develops effective relationships at all levels of the organization.</li><li> Participates in team concepts and promote a team effort; performs duties in accordance with company policies and procedures.</li><li> Regular and reliable attendance is required for the job.</li></ul>
