Construction Loan &amp; Closing SpecialistPosition Overview:BlueHub Loan Fund (BHLF), an affiliate of BlueHub Capital (BlueHub), seeks a Construction Loan &amp; Closing Specialist (CLCS) to support our community development lending program to a diverse set of borrowers. The CLCS will manage and maintain procedures and internal controls related to a complex set of commercial transactions in accordance with BHLF’s Loan Policies and Procedures.This newly created position requires a highly motivated person with a commitment to community development finance and, ideally, a background in construction management and monitoring, loan closing, or portfolio management. This person will be responsible for closing and funding loans in coordination with Loan Officers, the Portfolio Management Team, the Finance Department, outside consultants and legal counsel. They will also be responsible for reviewing construction requisitions, closing, disbursing and monitoring construction loans. identifies differences in goals of curative and palliative care; and provides appropriate education.</li><li>Respects patient’s wishes and stated goals when planning and providing care.</li></ul>Hospice Specific:<ul><li>Shares joint responsibility with other clinicians and volunteers to provide care to patients and families including psychosocial support, advanced directive information, development of Plan of Care, and other social services depending on the patient/family needs.</li><li>Attends and participates in Interdisciplinary Group (IDG) meetings; assists in developing and updating the Plan of Care; facilitates understanding of social services and bereavement aspects of care, and provides team with goals and interventions for patient/family care.&nbsp;</li><li>Evaluates bereavement needs including making condolence calls, evaluating risk and coordinating the Plan of Care with Bereavement staff.</li></ul>Palliative Care Specific:<ul><li>Assists the palliative care team and/or directly facilitates patient-family meetings for goal setting and advance care planning.</li><li>Provides emotional support for patients and families facing serious illness.</li><li>Coordinates educational and outreach services relevant to the social services – palliative care interface.</li><li>Provides liaison function: (1) between system-wide outpatient palliative care team and inpatient palliative care team, discharge planning and social service departments and (2) between palliative care team and community hospice, home care agencies, long term care facilities and other community services.</li></ul>Job Description Addendum for Social Worker in the Florida Keys &amp; North Florida (3A):Duties may also include performance in bereavement counseling and volunteer coordination.<ul><li>Evaluates the bereavement care needs for survivors (i.e., adults, children, families) in accordance with Company policy.&nbsp; Our employees make all the difference in our success!Benefits Day 1 Role: The Care Access Coordinator performs daily intake tasks that include, but not limited to, handling, processing, and responding to incoming telephone, fax, and email inquiries and referrals; completing outbound calls as practices specify. Responsible for scheduling of all referrals with admission staff and communicating with Clinical Liaisons in facilities to ensure follow-up of patients. Establishes and maintains positive relationships with team members, patients/families and referral resources; works in collaboration with the care access staff, insurance authorization staff, advanced practice clinicians, and Admission RNs to enhance the admission process.Qualifications:<ul><li>High school diploma or equivalent; Ensures medical staff’s face-to-face evaluation visit prior to the RN assessment.</li><li>Utilizes appropriate support/expert resources or personnel to resolve complex or difficult situations.</li><li>Adheres to expected productivity and accuracy targets established by management.&nbsp;
