Job Responsibilities: • Provides physical therapy treatment and interventions to patients based on POC established by Physical Therapist (PT) and under the supervision of the PT. • Provides education, training, guidance, and supervision to program staff and patients/family members regarding therapy. • Recommends therapeutic exercises to PT. • Participates actively in the Interdisciplinary Group (IDG) by providing clinical expertise in team planning, problem solving, and processes. • Fulfills all administrative requirements including, but not limited to, collaboration with other Therapists, attendance at team and staff meetings, and recording and tracking of physical therapy durable medical equipment (DME). • Provide treatment accordingly as approved by the Physical Therapist. • Records and signs treatments in patient’s medical record; records, signs, and dates clinical documentation according to Company policy and regulatory guidelines. • Notifies the Physical Therapist and pertinent others of changes in condition or the need for reassessment of the Plan of Care to meet patient’s needs.- Execute delegated physical therapy assistant tasks under supervision, supporting patient's progress. • Performs other job duties as assigned.This position requires consent to drug and/or alcohol testing after a conditional offer of employment is made, as well as on-going compliance with the Drug-Free Workplace Policy.
