We partner with health plans to provide a comprehensive suite of products and services designed to manage the total cost of care.

We are currently looking for a Care Enablement Coordinator to join our growing team!

The Care Enablement Coordinator will support the Nurse Care Coordinators in providing comprehensive care coordination services to patients. The Care Enablement Coordinator will consistently demonstrate excellent communication and organizational skills, and the ability to work with minimal supervision in a remote environment. The Care Enablement Coordinator will work closely with Nurse Care Coordinators and patients to ensure that patients receive timely and appropriate healthcare services.

Primary duties include, but are not limited to:

ESSENTIAL FUNCTIONS:

  • Identify patients needing care coordination.
  • Gather information from patients, families, and providers.
  • Monitor patient progress and adjust care coordination plans.
  • Maintain accurate patient records.
  • Participate in team meetings and communicate with all parties.
  • Collaborate with providers and organizations to connect patients to resources
  • Assist with solving challenges/barriers to scheduling, transportation, food insecurity, and durable medical equipment.
  • Escalate clinical needs as necessary.
  • Maintain up-to-date knowledge of healthcare landscape that could impact patient care.
  • Provide support and guidance on how to access health plan benefits and community resources to patients and families.
  • Collaborate with other members of the care team to ensure that patients receive coordinated and comprehensive care.
  • Collect data and report program outcome
  • Identify improvement opportunities in care coordination.
  • Engage in ongoing professional development.
  • Adhere to ethical standards and patient confidentiality.
  • Weekend/holiday coverage as needed

OFFICE LOCATION:

Remote

QUALIFICATIONS:

  • 2-3 years of healthcare experience, preferably in primary care, outpatient care coordination, or related roles.
  • Experience navigating health systems to obtain necessary DME, transportation, meals, and other resources for patients
  • Excellent communication and interpersonal skills.
  • Experience in gathering data and reporting outcomes.
  • Ability to work with minimal supervision in a remote environment.
  • Strong organizational skills and attention to detail
  • Proficiency in Microsoft Office and EMR software.
  • Initiative and strong problem-solving skills, the ability to work in a fast-paced environment, and a commitment to improving patient outcomes.
  • Passion for healthcare and improving patient outcomes.

KNOWLEDGE AND EXPERIENCE:

  • Bachelor’s Preferred but will accept candidates who possess relevant work experience that which aligns with the responsibilities and requirements of the position
  • Experience in care coordination, healthcare, or related roles.
  • Familiarity with community resources to support healthcare needs.
  • Proficiency in Microsoft Office, Outlook, Excel, Word, Adobe, Zoom, MS Teams, and EMR software.
  • Ability to work in multiple EMR systems simultaneously.
  • Initiative and critical thinking skills.
  • Commitment to patient-centered care.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively with a multidisciplinary care team.
