Responsible for executing the home health patients’ physical therapy Plan of Care (POC) as established by the Physical Therapist (PT) and the Physician’s orders. Delivers high quality professional physical therapy and education and performs therapy skills with a high degree of proficiency. Job Responsibilities: • Provides physical therapy treatment and interventions to patients based on POC established by Physical Therapist (PT) and under the supervision of the PT. • Provides education, training, guidance, and supervision to program staff and patients/family members regarding therapy. • Recommends therapeutic exercises to PT. • Participates actively in the Interdisciplinary Group (IDG) by providing clinical expertise in team planning, problem solving, and processes. • Fulfills all administrative requirements including, but not limited to, collaboration with other Therapists, attendance at team and staff meetings, and recording and tracking of physical therapy durable medical equipment (DME). • Provide treatment accordingly as approved by the Physical Therapist. • Records and signs treatments in patient’s medical record; records, signs, and dates clinical documentation according to Company policy and regulatory guidelines. • Notifies the Physical Therapist and pertinent others of changes in condition or the need for reassessment of the Plan of Care to meet patient’s needs.- Execute delegated physical therapy assistant tasks under supervision, supporting patient's progress. • Performs other job duties as assigned.This position requires consent to drug and/or alcohol testing after a conditional offer of employment is made, as well as on-going compliance with the Drug-Free Workplace Policy.
