Ayudar a encontrar soluciones a sus preguntas y/o reconocer la necesidad de una visita en persona. Coordinar la visita en persona cuando sea necesario o sea solicitado. • Ayudar al paciente y a su familia a alcanzar los niveles de confort deseados. • Utilizar recursos o personal de apoyo/experto adecuados para resolver situaciones complejas o difíciles. • Documentar la información de contacto del paciente/familia en el EMR y comunicarse con el Grupo Interdisciplinario (IDG). • Completar las admisiones clínicas para todos los servicios de la Compañía, incluidos, entre otros:o Explicar los servicios a los pacientes potenciales/familias y abordar las preguntas sobre los miedos del paciente, las limitaciones físicas, al tiempo que tranquiliza al paciente/familia; Presenta los servicios de manera empática y compasiva o Identifica objeciones de admisión que tenga el potencial paciente o familiares, preocupaciones u otras barreras de entrada y proporciona respuestas. o Educa al paciente/familia sobre los requisitos de admisión y aborda las necesidades inmediatas o Completa los consentimientos informados y la evaluación inicial de acuerdo con las regulaciones y la política de la empresa. o Proporciona información a los médicos e inicia un plan de atención para abordar las necesidades inmediatas del paciente. o Inicia intervenciones de enfermería especializada para prevenir complicaciones, aliviar los síntomas y maximizar la comodidad física y emocional. o Obtiene órdenes médicas o Completa la documentación según la política de la empresa• Actúa como representante de la Compañía en las instalaciones asignadas mientras facilita las referencias a todas las líneas de servicio; Trabaja en estrecha colaboración con los hospitales, los médicos, los centros, los pacientes, las familias y el público en general. • Se comunica frecuentemente con Admisiones para asegurar un seguimiento oportuno.
