The CCS Clinical Coordinator works under the general direction of the Nurse Manager and Director of Emergency Services. Is primarily responsible for conducting updated comprehensive assessments, brief crisis counseling, psycho-education as well as the development and implementation of the treatment team functions, therapeutic groups and individualized crisis prevention/safety/recovery plans/discharge plans. The CCS Clinical Coordinator will work as part of the CCS team to restore the person's functioning, to strengthen resources and supports for the individual and their family to ensure care coordination and discharge planning back to the community. Responsibilities also include clinical intakes and management of client admissions in collaboration with the Nurse Manager and ESP Director. The Clinical Coordinator will assist to ensure that client flow maintains budgeted census levels and that a structured yet "homelike comfortable" environment, conducive to recovery and well-being that is in compliance with the goals and philosophy of the Mass Health Performance Specifications is maintained.

ESSENTIAL FUNCTIONS:

1. In collaboration with the Nurse Manager, will mentor the Bachelor's-level milieu staff to treatment team functions, psycho-education groups, and individualized crisis prevention recovery plans.

2. In collaboration with the Nurse Manager, promotes a safe and structured recovery-based environment as outlined in the CCS specifications. Promotes crisis risk management.

3. Responsible for facilitation of treatment groups

4. Provides solution focused therapy

5. Ensures clients are provided with appropriate, strength-based, person-centered treatment interventions to best support the persons served in maintaining, entering or returning to a permanent living situation

6. Contributes to the comprehensive assessments and reviews clinical appropriateness of CCS admissions and discharges. Participates in morning rounds to contribute updated mental status, clinical treatment and discharge plans.

7. Reviews all potential admissions to CCS in collaboration with the Nurse Manager to ensure appropriateness for community-based clinical and stabilization services

8. Works in close collaboration with the Emergency Services team to promote continuity of care of client

9. Works with referral sources on CCS admissions and completes evaluations for step-down clients

10. Ensures that all applicable admission forms are completed, signed, and placed in the client's CCS chart in a timely manner

11. Formulates, implements and documents individual treatment plans with the assistance of external community treatment providers, client, and client's caregivers. Ensures implementation ad consistency of treatment plans by unit staff

12. Works in conjunction with the Nurse Manager in the areas of clinical services and client flow to maintain census at budgeted levels

13. Assists Nurse Manager with treatment team and staff meetings

14. Other duties as required by the Nurse Manager

Required Skills

  • Experience in crisis assessment strongly preferred
  • Bilingual skills and/or experience with multicultural populations preferred.

Required Experience

  • Master's Degree in Social Work, Mental Health Counseling, Psychology or related field
  • Massachusetts Independent License or License Eligible
  • 3 years of related experience including assessing and managing risk, crisis intervention and de-escalation techniques to promote stabilization of clients in crisis
  • Ability to work effectively within a team milieu and promote consistent team interventions and stabilization
  • Requires valid driver’s license, safe driving record, and access to personal vehicle for consumer transportation
