In this role, you will work under the direction of an Occupational Therapist and in collaboration with a multidisciplinary clinical team to implement therapeutic treatment plans, lead group activities, and support clients in achieving maximum independence and quality of life.The ideal candidate possesses strong clinical and organizational skills, thrives in a client-centered environment, and is adept at supporting individuals through behavioral, emotional, and physical challenges.Key ResponsibilitiesDirect Client Care & Clinical Interventions
    Treatment Implementation: Execute individualized treatment plans (ISPs/Treatment Plans) designed to enhance clients' functional independence, cognitive abilities, daily living skills, and coping strategies.Adaptive Equipment & Strategies: Assess, develop, monitor, fabricate, and repair adaptive equipment or compensatory strategies to promote full participation in daily tasks.Group & Leisure Programming: Design, organize, and facilitate culturally sensitive group activities and monthly schedules focused on cognitive stimulation, socialization, and recreational/vocational development.Behavioral & Emotional Support: Provide encouragement, manage difficult client situations (e.g., activity refusal or emotional distress), and teach constructive emotional regulation skills.Assistance & Care: Provide appropriate levels of physical assistance (minimum to maximum) with medication administration, medical procedures, transportation, transfers, dressing, and grooming. Support direct care staff when not engaged in clinical interventions.
Case Management, ISP Planning & Documentation
    ISP/Treatment Plan Management: Coordinate, complete, and distribute client assessments. Schedule, lead, and document annual ISP/Treatment Planning meetings and revisions, distributing updates to team members and families within required timelines (e.g., initial plan within 90 days of admission).Progress Tracking: Observe, evaluate, and record client progress, attitudes, and behaviors. Update client charts and maintain compliance with company standards and regulatory guidelines. Interdisciplinary Advocacy: Serve as a client advocate by communicating changes in client condition (behavioral, medical, emotional, cognitive, or physical) to team members, families, and payers.
Program Administration & Operations
    Staff Orientation & Training: Orient staff on clinical programming, client supervision, health and safety protocols, and off-site activities.Supply & Equipment Management: Collaborate with the Site Manager/Clinical Specialist to identify, order, clean, and assemble necessary therapy supplies and equipment.Facility & Safety Standards: Maintain cleanliness, safety, and regulatory compliance within the therapy environment.
Qualifications & Requirements
    Education: A high school diploma and three (3) years of related work experience along with active certification as an Occupational Therapy Assistant is required. Bachelor’s degree in healthcare or a related field is preferred.
    Skills & Competencies: Understanding of the needs of older adults and individuals with physical, developmental, or emotional disabilities (ongoing training participation required). Ability to solve problems collaboratively within a multidisciplinary clinical team.
    Operational Requirements: Valid driver’s license with a record of responsible driving and ability to lift up to 50 lbs.
