Certification of completion of Alzheimer's Disease and Related Dementias Training through the Florida Department of Elder Affairs.
Education and Experience
High school diploma or general education degree (GED) is required. Must have one or more year’s experience with a frail or elderly population. Experience working with outside agencies and customer service experience is preferred.
Certificates, Licenses, Registrations
Must possess a valid Florida driver’s license, current auto insurance, and reliable transportation.
Mathematical Skills
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
Reasoning Ability
Ability to solve problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Ability to communicate, concentrate and document effectively and accurately.
Language Skills
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.

Position Requirements (not all inclusive)
Responsible for the development and implementation of homecare services for program participants.
Assess the homecare needs of a frail elderly population, and identify and develop specific plans of care.
Collaborate with other disciplines to ensure that quality services are provided to meet participant needs.
Maintain an awareness of patient/family/caregiver needs and problems.
Coordinate and authorize all durable medical equipment, consumable supplies, environmental adaptations and other home services.
Coordinate scheduling of outside agency staffing and agency education, specific to PACE requirements.
Perform computer entry to include patient care and demographic data, timesheets, careplans and reports. Computer input is independently updated as needed and accurately entered in a timely manner. Reports, lists and other data are generated accurately and timely. Distribute all changes of information. Proactively and independently prepare reports. Anticipate needs based on activities of team.
Communicate and collaborate with members of the IDT in care and services to patients/families.
Demonstrate collaborative communication. Actively participate in Interdisciplinary team meetings.
Review invoices for PACE Home Services, monitor requisitions as appropriate.
Conduct annual Home Care Service Agency audit.
Process service requests for homecare needs as required. Facilitate coordination, delivery and pickup of DME/supplies. Seek assistance/clarification when needed. Vendor receives specific information and reports minimum dissatisfaction. Data entry is complete for items needed and ordered, date and initial order number, time needed and other special information. Patient/family/caregiver satisfaction.
Provide external/internal customer service. Communicate and collaborate with members of the IDT in care and services to patients/families in a friendly and positive manner. Provide assistance to staff members in locating documentation and/or information. Answer questions from the health care community appropriately.
Conduct unscheduled home visits as requested by IDT to ensure compliance with POC.
