Nexus Family Healing is looking to hire a Interim Temp Admissions and Transitions Coordinator to join our newest youth crisis and stabilization center in Hennepin County!


This new youth crisis and stabilization center is a 13-bed, short-term residential service is for Hennepin County youth with complex mental and behavioral health issues looking for appropriate care. The service will help to reduce the number of youth who are stuck in inappropriate care settings such as emergency rooms, police stations, juvenile detention or unstable home situations and provide treatment and therapeutic supports while triage, assessment, and transition planning take place with families.

Pay and Schedule:

  • Temp opportunity with full-time hours until further notice
  • Hybrid opportunity working in office and from the community.
  • Monday - Friday 8:30am - 5pm
  • Location: 1800 Chicago Avenue in Minneapolis
  • Pay Range: $22 - $24 hourly

Position Summary: Our Admissions & Transitions Coordinator serves as the primary contact for referrals, admissions, transitions, and discharge planning. This role coordinates intake, scheduling, insurance verification, transition and aftercare planning, and administrative functions while partnering with youth, families, providers, and community stakeholders to ensure timely access to services and continuity of care.


Primary Responsibilities:Admissions, Intake & Appointment Coordination
  • Respond to referrals and admissions inquiries; maintain client records in the EHR.
  • Coordinate admissions and facilitate timely placement decisions.
  • Collect, verify, and update client information and documentation.
  • Schedule services with qualified providers and manage appointments and related activities.
  • Obtain insurance authorizations and verify funding sources, as applicable.
  • Generate admissions and intake reports, metrics, and outcomes for leadership.
  • Manage medical records requests and documentation processes.
Transition, Aftercare & Discharge Coordination
  • Coordinate admissions, transitions, aftercare, and discharge activities to ensure continuity of care.
  • Collaborate with clinical teams, families, community partners, and stakeholders to develop person-centered transition and discharge plans.
  • Facilitate discharge planning from admission through successful reintegration.
  • Connect clients with services, supports, educational resources, and follow-up care.
  • Complete required documentation accurately and within established timelines.
  • Support treatment goals through effective communication, coordination, and discharge planning.
Office Support & Administration
  • Provide professional, welcoming customer service to clients, families, staff, and visitors.
  • Perform administrative duties supporting admissions, transitions, and program operations.
  • Coordinate meetings, schedules, communications, and other administrative functions.
  • Assist leadership and treatment teams with planning, organization, and program support activities.
  • Participate in meetings, training, quality improvement initiatives, and special projects.
Compliance, Communication & Professional Standards
  • Route incoming calls and messages to appropriate staff.
  • Identify and communicate operational issues affecting workflows and services.
  • Maintain compliance with agency policies, accreditation standards, HIPAA, Medicaid, and applicable regulations.
  • Build collaborative relationships with clients, families, referral sources, community partners, and staff.
  • Meet productivity, performance, and organizational expectations as a contributing team member.
  • Complete required training and stay current on policies, certifications, and job-related requirements.
Requirements:

Required Education and Experience:

  • High School Diploma or equivalent.
  • Demonstrated experience in a behavioral/mental health or healthcare office or equivalent.
  • Valid driver’s license required. Must meet state regulating agency and Nexus Home Office driving requirements

Preferred Education and Experience:

  • Associates Degree in Business, Office or Medical or Health Administration; or other Healthcare related field.
  • 2+ years of experience in healthcare administration, behavioral health services, intake coordination, admissions, discharge planning, case management, transition services, or related experience.
  • Electronic health record and healthcare billing experience in lieu of Associates degree considered.
  • Bilingual Spanish or Somali speaking is a strong asset.
