The Commitment Monitor Program is part of Multnomah County Community Mental Health Program Safety Net and serves adults that are managing severe persistent mental health disorders who are civilly committed in Multnomah County. The purpose of this position is to represent the Mental Health Authority in overseeing the course of care for a civilly committed individual while hospitalized, facilitate the least restrictive level of care for committee individuals as well as providing for the safety of the community. The services provided are defined in Oregon Statue 426 with further directions in Oregon Administrative Rule 309-033. This is an On-Call position assignment. Work hours are between 8:00 am - 5:00 pm Monday-Friday. Hours will vary but should be around 8-24 hours per week. This position is remote but does require in person visits to hospital psychiatric units. Many of the psychiatric units have a safety requirement that all staff and visitors must be able to walk without any assistive devices.
As theMental Health Consultant - On-Call, other duties will include:
Evaluation and Assessment
- Evaluate, review and approve for discharge from hospital, monitor progress of in-patient care, assess for meeting continued commitment criteria.
- Ensure discharge criteria includes appropriate community mental health appointments, housing, and entitlements are in place.
- Assess the need for extended hospital care through the Long Term Psychiatric Care referral process.
- Assess for compatibility for referral to Trial Visit Monitoring as a transition safety net for individuals discharging from the hospital.
Care and Case Planning
- Collaboration and consultation with community providers, hospitals, treatment teams and consumers' family/significant others.
- Solicit treatment recommendations from providers, work with providers to facilitate timely discharge.
Resource Planning
- Provide engagement, transition and community support services for clients.
- Recommendation and facilitation of outpatient treatment services, including housing.
- Connects clients to services and treatment that are available, best meet their needs and support recovery.
Documentation
- Create a Discharge Plan in Shared Monitor Files within three business days of the commitment.
- Accurately document client contacts, including the one face to face visit and then the follow up communications with the hospital and community providers for a minimum two contacts per week.
- Maintain current commitment and treatment information in the electronic health record (EHR) system and close files in a timely manner.
Court and Representation
- Represent the Civil Commitment Courts' interests relevant to committed clients.
- File all documentation including legal documents to court and electronic health record system in accurate and timely manner.
- Complete Involuntary Investigations including completing Hearing Reports for Protested Recertification of Civil Commitment.
- Provide testimony at court appearances for Civil Commitment Hearings, Recertification of Civil Commitments, Revocation of Trial Visit Hearings, or other legal issues as required.
- Process and approve termination and/or transfer of individuals on Civil Commitment status as appropriate.

MINIMUM QUALIFICATIONS (REQUIRED):
Education/Certification: Master's degree from an accredited college or university in the mental health field with major coursework in social science, or a related field demonstrating the capacity for the required knowledge and skills to meet provider Qualified Mental Health Professional (QMHP-C) credentialing requirements as required by Oregon Administrative Rules (OARs) and the Mental Health & Addiction Certification Board of Oregon (MHACBO) - more information can be found at MHACBO website. Current QMHP-C, and/or licensed Board of Oregon (LPC, LCSW, PHD, etc) See qualified degrees listed below:
- Master degree in psychology.
- Master degree in social work.
- Master degree in a behavioral science field.

Preferred Qualifications/ Transferable Skills*: You do not need to have the following preferred qualifications/ transferable skills to qualify. However, keep in mind we may consider some or all of the following when identifying the most qualified candidates.
- Licensed Board of Oregon (LPC, LCSW, PhD,etc) (preferred).
- Knowledge of challenges and barriers faced by those with Severe and Persistent Mental Illness (SPMI) and the systems they encounter in Residential Services, Adult Protective Services and Crisis Services.
