Physical Requirements

The physical requirements for this position include occasionally lifting, pushing, and/or pulling objects weighing up to or over 50 lbs.; regularly sitting at a computer workstation for extended periods of time; and some job-related travel.

Monday through Friday, 8:00am-5:00pm

Job Summary

This position will work closely with both inpatient and outpatient providers of the Advanced Heart Failure service to improve care coordination for the heart failure population.

Skills / Knowledge / Abilities

The ideal candidate for this position will have extensive disease-specific knowledge; the ability to work independently and think critically; will be well-organized and detail-oriented; and will have excellent communication skills.

Preferred Education/Experience

Previous experience in cardiology and the care of heart failure patients preferred.
