In this role, you will lead the nursing assessment, planning, coordination, and evaluation of medical and rehabilitation services for our clients. Working closely with Rehabilitation Case Managers (RCMs), physicians, and interdisciplinary treatment teams, you will play a vital role in helping clients achieving their healthcare and rehabilitation goals following chronic or catastrophic illness/injury.As a supplement to the essential duties and responsibilities outlined in the Registered Nurse job description, the RN role in Collage's Colorado program will also be responsible for the following case management tasks and functions:
    Maintain a case management caseload, and be responsible for all case provisions for a caseload, including residential neurorehabilitation services, supporting living, and day treatment clientsFacilitating funding communications, providing clinical updates, discharge planning information, fostering partnership and addressing any identified issues as needed.
Essential Duties & Responsibilities Clinical Assessment & Care Planning
    Conduct comprehensive nursing assessments to evaluate client strengths, challenges, functional status, and healthcare goals upon admission.Analyze medical referrals, personal histories, and diagnoses to establish targeted, priority nursing care plans.Develop short- and long-term treatment goals through interprofessional collaboration and resource management.Assess, secure, and monitor adaptive equipment and strategies tailored to each client’s rehabilitation and discharge plan.
Interdisciplinary Coordination & Direct Care
    Collaborate with RCMs, physicians, and external healthcare specialists to procure and coordinate targeted health and social interventions.Provide direct clinical care, medical triage, and support during client appointments or at program locations as needed.Lead psychoeducational or health-related group sessions (e.g., brain injury education).Manage and support clients in challenging situations, including medication or activity refusal.Participate in an on-call rotation to address urgent clinical needs.
Education & Advocacy
    Educate and train clients, families, support systems, employers, and funding source representatives on treatment and discharge plans.Train team members on healthcare strategies and nursing care plans to ensure consistent implementation across all shifts.Contribute to new staff orientation and lead ongoing training on health-related topics.
Documentation & Administrative Management
    Maintain timely and accurate medical documentation, including nursing progress notes, medication records, lab work tracking, and assessment evaluations.Record client contacts and service duration in the Client Information system in accordance with established timelines.Actively participate in team meetings, medical rounds, and discharge planning discussions.
Qualifications & Requirements
    Active State Licensure as a Registered Nurse (RN) is required (BSN preferred).Minimum of 2 years of related clinical nursing experience required.Prior experience in the rehabilitation of catastrophically or chronically ill/injured populations (e.g., brain injury, complex trauma) is strongly recommended.Certified Rehabilitation Registered Nurse (CRRN) or related specialty certification is highly recommended.Valid driver’s license with a record of responsible driving behavior and ability to lift up to 50 lbs.
