Job Summary
- The RN-Clinical Coordinator is responsible for coordinating multidisciplinary care across various departments, including ICU, Medical, Surgical and Ancillary Units, ensuring quality patient care delivery
- The role involves collaborating with nursing staff, physicians, and ancillary departments to assess, plan, implement, and evaluate patient care while supporting quality improvement initiatives and regulatory compliance in an acute care hospital setting
Essential Functions
- Coordinate patient care across ICU, Medical, and Surgical Units to ensure optimal patient outcomes and continuity of care
- Collaborate with interdisciplinary teams to assess, plan, implement, and evaluate patient care plans
- Provide clinical guidance to nursing staff, serving as a resource for clinical procedures, protocols, and hospital policies
- Participate in quality improvement initiatives, monitoring patient outcomes and implementing corrective actions as needed
- Conduct daily patient rounding to assess patient needs, monitor care quality, and provide support to nursing staff
- Facilitate communication between medical staff, patients, families, and ancillary departments to ensure effective care coordination
- Assist in discharge planning, ensuring patients receive appropriate follow-up care instructions and resources
- Develop and implement educational programs to enhance clinical competencies and promote adherence to best practices
- Monitor compliance with regulatory guidelines, including Joint Commission, EMTALA, and hospital-specific protocols
- Document patient care activities accurately in the electronic medical record (EMR) and other clinical systems
- Perform other duties as assigned by the Quality Director or Chief Nursing Officer

- Proficient in Microsoft Office applications (Word, Excel, Outlook)
- Ability to prioritize tasks and manage multiple responsibilities effectively
- Demonstrated knowledge of regulatory standards and quality improvement processes
- Ability to work effectively in a fast-paced, multidisciplinary team environment
Qualifications
Education
- Associate Degree in Nursing (ADN) required
- Bachelor of Science in Nursing (BSN) or Master’s in Nursing, Healthcare Management, or Administration preferred
Licenses/Certifications
- Current RN licensure in the state of practice or compact state required
- Basic Life Support (BLS) certification required within timeframe specified in facility policy
Experience
- Minimum of 2 years of acute care inpatient nursing experience required
- Experience in patient care coordination, quality improvement, and leadership preferred
