Job Summary
The STEPS Referral Coordinator plays a key role in supporting patients’ access to Food as Medicine programs. This position assists individuals with completing enrollment paperwork, ensures required program steps are completed daily, and helps resolve barriers that may impact participation. The Referral Coordinator also provides problem-solving support related to financial or social challenges, transportation needs, and basic technology issues, ensuring patients receive seamless, equitable access to program services.
Key Responsibilities Include:
- Assist patients with completing program enrollment forms and required documentation
- Guide patients through daily steps needed to participate in Food as Medicine programs
- Identify and troubleshoot barriers related to finances, social needs, transportation, or basic IT issues
- Maintain accurate records in REDCap or other data management systems
- Ensure strict adherence to HIPAA and other privacy regulations
- Communicate professionally with patients, staff, and community partners
- Collaborate with team members to support program operations and continuous improvement
- Perform additional duties as assigned
Skills / Knowledge / Abilities
- Strong organizational skills with excellent attention to detail
- Proficiency in Microsoft Office; familiarity with REDCap or other data management platforms
- Excellent verbal and written communication skills
- Strong problem-solving abilities and ability to work independently when needed
- Ability to follow instructions and comply with all HIPAA guidelines
- Demonstrated ability to work effectively in a collaborative team environment
- Ability to engage positively and professionally with diverse community members
