Brief Job Overview/Summary

Description POSITION SUMMARY The Registered Nurse is a member of the interdisciplinary team working in collaboration with the Physician. The Registered Nurse utilizes the nursing care process consistent with their license and/or certification including initiation of culturally competent continuum of care including inpatient, outpatient and community settings. The Registered Nurse is responsible to oversee any necessary coordination of the interdisciplinary plan of care and provides for its consistency with the medical treatment plan. PERFORMANCE EXPECTATIONS The case manager will work closely with clinical staff, peer support specialists, health care providers, courts, probation, and community partners. They will participate in the multidisciplinary team meetings and case conferencing and will advocate for client needs across system and help reduce barriers to treatment. May require occasional evening or weekend hours depending on program needs.This position requires repetitive motion, hearing, pushing, reaching, and reading, seeing, speaking, standing and walking a majority of the time. Spends time with such equipment as computer terminal, fax machine, copier, and telephone. Occasional driving also required.
