Additional Duties: <ul><li>Provide outreach, education, and case management to high-risk perinatal &amp; postpartum women, and infants up to one year of age through home visits.</li><li>Conduct home visits for prenatal, postpartum, and infant assessments to assess patient conditions and coordinate with providers to ensure appropriate care.</li><li>Educate patients, offer referrals for additional services, and document observations and follow up plans.</li><li>Attend required meetings (local, District, and State).</li><li>May supervise additional staff as needed.</li><li>Willing to travel within District 2 as necessary.</li><li>Familiar with OB/GYN hospital practices</li><li>Knows vaccine schedules for children and adults</li><li>Gives correct needed vaccines to children and adults</li><li>Gives vaccines needed for school attendance along with correct certifications</li><li>Other duties as assigned</li></ul><dt><h4>Minimum Qualifications</h4></dt>PH RN 1: Associate's degree in nursing from an accredited college or university AND completion of a PHN course within one year of hire AND current Georgia Registered Professional Nurse license AND CPR certification.
