Make a difference right in your own community by helping those struggling with recoveryDUTIES AND RESPONSIBILITES:<ul><li>Oversee coordination of a patient’s care while in PHP, including daily group therapy, 2 individual sessions per week and family/referent updates as needed.</li><li>Complete clinical assessments (level of care evaluation, biopsychosocial/integrated summary), develop person-centered plans, provide therapeutic intervention and review goals biweekly to determine level of care necessity.</li><li>Aftercare assessment and planning – assess unresolved and deferred problems and write goals in the aftercare plan and coordinate referral to appropriate aftercare providers.</li><li>Oversee the collection of rand urine drug screen and breathalyzer samples from patients and relay results.</li><li>Responsible for documenting treatment plans, progress notes, treatment plan updates, case consultations, telephone calls and discharge summaries in the patient records in a timely fashion.</li><li>Facilitates group therapy and individual counseling that is consistent with the patient’s treatment goals and their treatment plan.</li><li>Family updates and sessions as clinically appropriate</li><li>Responsible for completing necessary utilization review information and forwarding it to the Financial Resource Specialists.</li><li>Works within the ethical constraints of the certified addiction counselor and other professional or organizational ethical standards as they apply to the position.</li><li>Document treatment plans, progress notes, treatment plan updates, case consultations, telephone calls and discharge summaries in the patient electronic health record in accordance with Caron policies and procedures.</li><li>Completes necessary utilization review information.</li><li>This job description reflects management’s assignment of essential functions; it does not prescribe or restrict the duties or tasks that may be required or assigned.</li></ul>&nbsp;PHYSICAL REQUIREMENTS:The physical and mental demands described here are representative of those that must be met to successfully perform the essential functions of this job.
