Primary role will be to assist the PACE primary care providers and interdisciplinary teams (IDT) in the coordination of and quality assurance for transitions of care to PACE contracted institutional settingsIncludes a $10,000 Sign on bonus!In this role, the Nurse Liaison will:<ul><li>Monitor care transitions for admissions and discharges to and from all &nbsp;PACE contracted settings, to include acute and chronic hospitals, psychiatric care units, inpatient hospice, and nursing facilities.</li><li>Serve as a liaison to PACE PCPs and the IDT in this coordination role.</li><li>Serve as an educator, counselor, and advocate for families and participants with regard to treatment location options and discharge planning decisions.&nbsp;</li><li>Establish on site and off relationship with contracted hospital facility and other partners regarding eligibility, approval, and authorized locations for post acute care</li><li>Assist the IDT and the PACE quality team in addressing transition care &nbsp;in the facilities, primarily in SNF settings</li></ul>EDUCATION: <ul><li>RN with BSN or related baccalaureate degree highly preferred. Explains and provides information regarding disease and wellness, tests, results, medications, diet, exercise, and other aspects of the plan of care to patients.</li><li>Safely and effectively administers medications, tests and treatments as ordered. Manages the refills of all patient medications with appropriate authorization by the provider.</li><li>Provide and coordinate health teaching required by individuals, families and groups to maintain the optimal possible level of health.</li><li>Completes biannual and status change nursing assessments for an assigned panel of participants in accordance with Care Planning Policy and documents problems, goals &amp; interventions in the participant’s care plan.</li><li>Utilizes Assessment tools including but not limited to PACE Nurse Assessment and Medication Management to collect data.</li></ul>Qualifications and Requirements:<ul><li>Baccalaureate degree in nursing preferred.
