The UC PMHNP is responsible for providing quality patient care under the guidance of the Medical Director of the Urgent Care and in accordance with departmental policies and procedures.  The UC PMHNP reports to the Medical Director of the BH UC or designee.<br><br>The UC PMHNP will provide emergency psychopharmacologic evaluations and prescriptions to adults and children presenting to the BHUCS in collaboration with a multidisciplinary team.<br><br>This position is part of a team that staffs the Behavioral Health Urgent Care Service (BHUCS) 24 hours a day, 7 days a week and as such will include shifts that cover that time frame.<p>Establishes rapport and develops therapeutic relationships with patients of diverse backgrounds and varying levels of motivation for change.</p><p>Writes up and presents completed emergency assessments for clinical disposition in a clear and organized manner.</p><p>Evaluates the psychopharmacologic needs of the BH UCS patients and provides short-term prescriptions when necessary.</p><p>Demonstrates knowledge of the full range of community resources to property guide and direct patients.</p><p>Provides consultation to and coordinates patient care with multidisciplinary BHUCS staff and the staff of the Satellite Emergency Facility..</p><p>Conducts a focused physical exam when necessary.</p><p>Orders and interprets appropriate laboratory tests, radiologic tests and EKGs with the support of the SEF.</p><p>Demonstrates the capacity to diagnose and effectively treat a range of behavioral health conditions: anxiety, depression, adjustment disorders and moderate-severe personality disorders.</p><p>Demonstrates excellent, advanced phone assessment/triage skills, assigning the appropriate level of priority at all times.</p><p>Performs advanced, high-quality, thorough psychosocial evaluations that contains data in support of the diagnosis, findings, recommendations and plans.</p><p>May obtain X waiver and prescribe medications to assist with substance use recovery.</p><p>Provides consultation, training and supervision to departmental staff as well as interns and PMHNP residents.</p>

Must be able to work independently and efficiently in minimally supervised setting, problem solve effectively while recognizing when to seek advice from supervisory staff, and  show respect and appreciation for the participant’s home, guests, and community. May be assigned to other supervised settings such as the PACE CenterEssential Duties and Responsibilities:<ul><li>Consistently monitors participant ADL/IADL care plans and follows plan of care as stated.</li><li>Documents care provided on participant flow sheets.</li><li>Check the health aide voicemail at the beginning of each shift and every two hours until the end of the shift. Reports any abnormal findings, alterations in participant status and/or care refusal to the team in a timely fashion by reporting to the appropriate day center call in line or Nurse On Call when applicable.</li><li>Assists participants with ADLs per assignment, as defined in ADL/IADL care plan and as needed, for example when a participant requests assistance with toileting.</li><li>Accompanies participants to appointments as assigned.</li><li>Carries out simple treatments under the direction of nursing staff, including simple catheter care, skin assessments during personal care.</li></ul><p>Qualifications and Requirements:</p><ul><li>High School graduate or equivalent preferred.</li><li>Current certification as a H.H.A. /C.N.A. from an approved program. (ex. Red Cross).
