Other duties may be assigned.&nbsp;<ul><li>Coordinates the ongoing Individual Service Plan (ISP) process, to include thorough assessment of individuals served, coordination of ISP team meetings, development of a comprehensive ISP, ongoing reviews to ensure services and supports are provided in accordance with the ISP, verifies the ISP is designed to ensure health, safety and welfare, reflect current level of needs and services as based upon the results of the assessment process and expressed preference of individuals served.&nbsp; Ensures achievement of outcomes that are important to the individual and outcomes that are important for the individual are included in the ISP.&nbsp; &nbsp; &nbsp; &nbsp;As the team leader, addresses the balance of and any conflicts between what is important to the individual and what is important for the individual.&nbsp; &nbsp;</li><li>For assigned cases, coordinates ISP meetings in keeping with the philosophy of self-determination, plans services to meet assessed needs through one single unified ISP.&nbsp; Develops an ISP that supports the concepts and principles of self-determination in accordance with standards as defined through the Ohio Department of Developmental Disabilities rules and MCBDD policies and procedures.&nbsp; &nbsp; &nbsp; These self-determination principles and concepts will include individual’s choice of service providers, development of individuals budgets/personal resources, and individual control of services to the extent feasible for each person served.&nbsp;</li><li>Completes the writing of Individual Service Plans (ISPs) within the guidelines established by the SSA department so that the ISP is distributed to the team prior to service delivery date.&nbsp; communicates service dates, corrections, suspensions, disenrollment to Waiver Administration, coordinating changes, ISP revision/reviews, and preventative measures when applicable.&nbsp;</li><li>Will comply with all local, State, Federal rules regulations, statutes, and standards, including DODD and CMS interpretation of said rules.&nbsp;</li><li>Provides support and solicits active participation by the individual and significant others of their choice in the planning process.&nbsp;</li><li>Recommends as a team leader on each assigned individual’s ISP team, or a Medina County DD Board representative for an ICF (as assigned), so that a unified community plan of service can be developed.&nbsp;</li><li>Acts as primary point of coordination for person and between team members for the ISP. Attends meetings, hearings, etc. at sites located throughout the county and state. Assists individuals/families to exercise their rights (due process, access to financial rights and benefits, etc.).&nbsp; &nbsp; &nbsp; Provides feedback regarding the results of the ongoing service review process to individual/family and other appropriate parties. Assists the individual and their family with negotiating services, solving problems, addressing barriers, and making adjustments to the ISP as needed.&nbsp;</li><li>Promote the involvement of agencies/individuals to arrange services that significantly impact the life of the individual while eliminating duplication.&nbsp; Assist the individual in obtaining appropriate agency or community-based services.&nbsp;</li><li>Maintain records and reports in a timely and accurate manner, consistent with accreditation and regulatory standards or contracted agreement.&nbsp;</li><li>Complies with guidelines established by DODD and the Medina Board of Developmental Disabilities regarding reporting of alleged abuse and/or neglect and other incidents as outlined in OAC 5123:1-17-02: Major Unusual Incidents.&nbsp;</li><li>Maintain contact with the person served and all ISP team members in addition to maintaining effective working relationships with staff of other agencies and facilities, families, and the public.&nbsp;</li><li>May arrange for transportation for individuals or may provide transportation as a last resort.</li><li>Provides consultation to staff on specific cases.&nbsp;</li><li>In accordance with DODD requirements, will be responsible for documenting daily and submitting monthly Medicaid Waiver billing documentation in accordance with the Targeted Case Management and / or Medicaid Administrative Claiming Guide. Hours are subject to change due to business need.Qualifications:A successful candidate will possess the following qualifications:<ul><li>Minimum of 1 year recent experience servicing premium customers</li><li>Airline, lounge or travel industry experience preferred, including reservation experience</li><li>Passion for servicing customers with proactive solutions</li><li>Exceptional communication skills, delighting the customer with every single interaction</li><li>The ability to listen to the customer’s needs, in turn offering confident recommendations, resulting in high satisfaction from the customer with the service they received</li><li>Must exhibit the utmost professionalism and maintain a positive attitude in all interactions</li><li>The ability to rely and collaborate with your colleagues is essential, as the lounge is a very active, fast-paced, team environment</li><li>Ability to make decisions quickly and effectively, when unanticipated situations may arise, putting the Card Members’ experience at the forefront of everything you do</li><li>Must be resourceful and capable of using the Internet effectively while providing assistance in a face to face customer environment</li><li>Ability to utilize and switch from multiple technology platforms with ease</li><li>Adapt to an ever changing environment and being resilient</li><li>Ability to work flexible shifts including nights, weekends, and holidays is essential</li><li>Light to moderate lifting may be required for certain job tasks</li></ul>Salary Range: $24.00 to $25.25 hourly + bonus + benefitsThe above represents the expected hourly pay range for this job requisition.
