Position: Qualification Coordinator IIDepartment: Referral ManagementReports to: Referral Management SupervisorPosition Description:The Qualification Coordinator II is responsible for accurately processing all assigned referrals. The Qualification Coordinator II acts as a liaison to facilitate communication between referral sources, patients, and the health care agencies.
